Job description

Mersey Care Foundation Trust are delivering an award winning, dynamic and expanding Multidisciplinary Community Service across Southport & Formby known as ICRAS (Integrated Community Reablement Assessment Service). ICRAS is focused on delivering the best of care and reducing the risk to adults including frail elderly and those who may be in a health or social care crisis, improving quality of care in line with current evidence. ICRAS also provides bed-based services within local residential and nursing homes, domiciliary care and urgent care response.

We wish to recruit an enthusiastic and experienced Physiotherapist who has a clear understanding of the role of rehabilitation and admission avoidance within a community setting and bed based setting, working predominantly with older people. Candidates should demonstrate the ability to work in a team which delivers innovative, responsive services to meet both the needs of patients and the organisation. We are committed to staff development and you will have a named supervisor, the support of other therapists, including clinical leads, occupational therapists, physiotherapists and nurses as well as the opportunity to attend courses and training.

The therapy service is delivered 8am – 8pm over 7 days. The successful candidate must be willing to assist the service meet the needs of it’s patients and the wider health economy by participating in weekend work and late shifts when required.

The Specialist Physiotherapy role will function as part of the multidisciplinary team within ICRAS to provide a seamless service to patients. The post holder will be expected to effectively manage a caseload of patient’s co-morbidities as an autonomous practitioner, using evidence based patient centred principles to assess, plan, implement and evaluate interventions of the patient group. The post holder will provide leadership for junior staff, when appropriate, through supervision and appraisal. The post holder will participate in planning, development and evaluation of services provided, holding responsibility for defined service improvement projects.

The post holder will take a holistic approach in the assessment and treatment of patients, being responsible for the provision of appropriate, high quality therapy and care to a caseload which may be of a complex nature, using the appropriate model of care and evidenced based practice.
Work in partnership with other professionals to enable patients to achieve physiotherapy goals in an appropriate environment, thus avoiding acute hospital admission, reducing risk of falls and facilitating discharges from the acute setting. The post holder will participate as an active member of the multidisciplinary team in developing and delivering high quality and innovative services to patients. You will promote effective teamwork, delegating activities appropriately, to the benefit of the patient and service delivery.

The successful candidate will have the opportunity to work alongside a range of highly skilled health professionals from a range of disciplines, including Frailty Practitioners and Advanced Nurse Practitioners (ANPs). This allows great opportunity to learn about different aspects of the service, continuously developing skills and knowledge.
