An exciting opportunity has arisen, are you passionate about the delivery of high quality palliative care to people living and dying with dementia?

We now require an enthusiastic and passionate Dementia Palliative Care Assistant, to fulfil an integral role within the Dementia Palliative Care Team covering the area of Derby City.

You will be required to travel to various sites across Derbyshire, therefore having a current full driving licence or means to get between the sites is essential.

Base will be Florence Nightingale Hospital, Derby.

As this post may involve care home access we will need to see proof of your COVID vaccination status (double vaccinated) for successful appointment to this role.

You will work as an integral part of a small team supporting people with dementia and palliative care needs (and their carers) across Derbyshire.

The primary goal of the team is to enable improved palliative care for people living with advanced dementia, with mental and physical health needs in care homes/supported living or their own home.

Alongside the registered clinicians, the post holder will support the delivery of palliative and end of life care in an individualised and person-centred way within people’s own homes and care homes.

The team is be focussed on ensuring the person remains at home (wherever home is) and avoiding unnecessary acute care admission.

The team will also support discharges from acute hospital beds and pathway 3.

This team has developed as a collaborative resource in response to a gap in the current pathway of care for people living with advanced dementia with palliative care needs.

Training and supervision will be provided to enable individuals to develop their skill set, and integrated working within the team will support case management.

Our purpose is to provide community health services to a patient population of over one million people in Derbyshire and Derby, as part of Joined Up Care Derbyshire.

We employ around 4,200 substantive staff, caring for patients in 11 community hospitals and more than 30 health centres, as well as in clinics, GP practices, schools, care homes and, increasingly, in people’s own homes and via virtual consultations.

During the pandemic response we have led the implementation of public vaccination centres in collaboration with partners across health and social care.

Under the supervision of qualified nursing staff:

  • To provide palliative care and support to people living and dying with dementia in accordance with individual Dementia Palliative/EOL individualised care plans.
  • To promptly report all observed changes in patient conditions, accidents, incidents and other important information to the allocated Nurse and take action as directed.
  • To provide palliative care and support in such a manner that the person’s individuality, dignity and privacy are respected where this does not compromise safety.
  • To deliver therapeutic care by means of supporting people/families/carers in the creation of Life Stories, Memory Books and This is Me’s.
  • Communicate effectively and sensitively and use language appropriate to patient and carer/relatives condition and understanding.
  • Effectively use all forms/methods of communication and be aware of and manage barriers to communication.
  • To assist in the maintenance and management of the supplies and equipment and to report any problems to the appropriate team member.
  • To attend and participate in any training courses and events as directed by Senior DPC Nurse/Clinical Lead.
  • To participate in the Trust’s staff appraisal system and supervision network.
