Job Description.

My client, a Northwest ICB requires an interim CYP CHC assessor to start immediately to cover a period of integration and change.

Day to day duties:

Provide support for Children and Young Peoples continuing care assessments
Responsible for new referrals, transition cases and reviews
Managing complex caseloads, ensuring thorough assessments
Responsible for brokerage and care package initiation and dealing with complaints and appeals
Present cases for monthly panels and agree bespoke packages
Support children with various care needs to include care at home, in school, hospice, voluntary care organisations and school holiday clubs
Contributed to and influence the management of specific care groups of children and young people
Undertake co-location working with colleagues in SEND and Social Care Management
Support family friendly services and other community services
Provide clinical expertise and knowledge to support the decision-making process
