We have a number of fixed term contract posts for a Band 5 CHC Practitioner, working across various sites around the Midlands and Lancashire regions including Lancashire, Cumbria, Merseyside, Southport, Staffordshire, Derbyshire, Leicestershire and Shropshire. This is an opportunity to join a new centralised review team. Duties will include reviewing patients with complex care needs who are eligible for NHS Continuing Healthcare funding or NHS-funded nursing care.

The posts are agile with a requirement to attend the office as and when required and there will also be a requirement to complete some assessments face to face.

A vehicle for the purpose of business travel is essential for this role.

The successful candidate will provide specialist assessment and review for adults and older people presenting with long-term care needs who may be eligible for NHS Continuing Healthcare, NHS-funded nursing care and jointly funded packages of care in line with the NHS Framework for Continuing Healthcare.

Experience of undertaking assessments, and working with patients, families and carers is essential. The Personalised Healthcare Team are responsible for the assessment and review of people who reside in nursing care homes and the clinical commissioning of packages of care for people with continuing healthcare needs who reside in their own homes.
