The role - We are looking for professionals with a proactive nature and excellent organisational skills to join our hardworking team. You will conduct assessments for NHS Continuing Healthcare and NHS-funded Nursing Care, working to realistic deadlines and proactively case manage individuals who are eligible to receive NHS funding. Our aim is to deliver a consistently excellent client service.

Your Skills, Experience and Qualifications:
Registered health professional with current professional registration.

Strong person centred attitude to service delivery; you are friendly, personable and confident when liaising with others.

Outstanding communication skills, an ambitious attitude and a precise attention to detail, you're happy to prioritise your own workload and complete tasks without supervision.

Previous experience within NHS Continuing Healthcare would be desirable but is not essential. However a willingness to learn new skills and put people first is essential.

Proficient in Microsoft Office.
Eligible to work in the UK.

In possession of a full driving licence and own transport; you will be required to work across Norfolk and Waveney and further afield if necessary.

This is a brilliant opportunity for individuals looking to develop their existing clinical assessment skills within friendly, supportive teams with opportunities to work in a more integrated way with our Local Authority colleagues.

  • To clinically assess patients for NHS Continuing Healthcare eligibility in patients’ homes, care homes and acute/community hospitals in the locality served by the CCGs of Norfolk and Waveney.
  • To follow the process laid out in the National Framework for NHS Continuing Healthcare; to include organisation, co-ordination and participation in MDT eligibility recommendations to be verified by CCG decision makers.
  • Ensure assessments conducted are robust, transparent and multi-agency (where required) for NHS Continuing Healthcare/NHS-funded Nursing Care
  • Analysis of supporting documentation to establish critical history of the patient’s relevant needs to produce a specialist nursing assessment.
  • Complete new referrals for NHS Continuing Healthcare within 28 days as per the National Framework for NHS Continuing Healthcare.
  • To undertake NHS-funded Nursing Care (FNC) determinations for service users who require ongoing nursing care in a care home with nursing, ensuring that patients are assessed within current guidelines and then reviewed after three months/ annually in line with DoH framework throughout the locality area served by Norfolk and Waveney CCGs.
  • Identify service users whose care needs could be met in a more appropriate setting. Initiate the process to implement this, for example Discharge to Assess, Palliative Care, Intermediate Care, Rehabilitation and District Nursing.
  • Carry out joint assessments with Social Service colleagues whenever requested to ensure appropriate placement of service users.
  • Identify (when relevant) service users who are not fit for discharge and require further medical/rehabilitative intervention.
  • Support community/NSFT hospital ward nurses to deliver safe health funded discharges. At times, when directed to do so, this may also include in-reaching into the acute setting to complete Norfolk and Waveney’s Discharge to Assess model of practice for complex discharges.
  • Ability to manage challenge and conflict in regard to NHS Continuing Healthcare processes and recommendation/decision making.
  • Be influential within a complex and challenging environment.
  • Liaising and sharing information with other teams on a ‘need to know’ basis where appropriate and with permission to do so.
  • Can be required to impart distressing information to families regarding placement and future care including discussion of poor prognosis. This may include completing referrals for Fast Track where clinically deemed appropriate.
  • Advise and influence resolution with any parties involved in conflict regarding the assessed outcome of care.
  • Dealing and working with people with complex needs and challenging behaviour, and people approaching end stage of life.
  • Dealing with difficult family situations and dynamics which require a high level of competent effort, and the ability to remain objective and use reflective practice and team support.
  • To work across the hospital and care home interface.
