The Liaison Care Delivery Team
As we continue to grow, we are looking for Care Practitioners for Continuing Healthcare (CHC) and Adult Social Care reviews and assessments who want to be part of a vibrant team who are making a difference. We welcome applications from people who want to work flexibly, full or part time, or to work around other commitments or family. We will consider people who have busy lives and cannot commit to full time, as perhaps you work already. This would include registered nurses and social workers who want to work 3 or more days per week.

Main responsibilities
Working within the National Framework for NHS Continuing Healthcare (CHC) and / or the Care Act 2014 you will chair Multi-disciplinary Meetings (MDTs), and complete reviews and assessments on behalf of Integrated Care Boards (ICB). These will be done with both the individual, the provider and their family (or their representatives) and will be held virtually on MS Teams or Zoom.

Communicates very sensitive, complex condition related information to individuals, relatives where there may be barriers to understanding requiring persuasive and influencing skills, whilst demonstrating empathy and reassurance.- Excellent verbal and written communication skills
- Highly skilled in engaging others in collaborative working through effective negotiation, persuasion, and interpersonal skills.
- Being able to deal with highly sensitive, highly complex information in relation to changes to service provision
- Excellent written communication skills with the ability to produce and present reports to clinical and senior management level audiences, often in relation to complex or contentious subject matter
- Analytical and judgement skills for assessing and interpreting specialist conditions.
- Planning and organisational skills
- Work to KPI’s
- Liaise with other services and professionals both internal and external.
- To plan provision of care for individuals
Essential:
Registered Level One Nurse with NMC or Social Worker must be Social Work England (SWE) or Scottish Social Services Council (SSSC) registered- 12 months post qualifying experience
- Operational Continuing Healthcare experience with completion of Decision Support Tools (DSTs) and or CHC reviews.
- Administration, Clerical experience
- Computer literacy, specifically Microsoft suite to include Outlook & Excel
- Knowledge of CHC National Framework 2022, Care Act 2014 and Mental Capacity Act 2019
Desirable:
- Experience in working to Key Performance Indicators (KPI’s)
- Project management experience
- Working in multidisciplinary environments
- Person centred care and support planning
- Experience of working with Personal Health Budgets and Direct Payments
