Job description
The Complex Case Manager will work across care pathways for patients with complex specialised neuro rehabilitation and spinal cord injury (SCI) needs in Sussex. Provides oversight, assessing patients and supporting referrals to and from specialist services.
Additionally, the role will work with providers to resolve ‘bottlenecks’, provide advice and recommendations on alternate service providers as needed, monitor adherence to specified pathways of care, including discharge to alternate care, local hospitals, step down services or to home.
The postholder will be responsible for managing the patient pathway, ensuring review all of referrals and placements; attendance at case reviews and holding providers to account for the effective flow of patients throughout the commissioned pathway. Where necessary the postholder will work with providers commissioned by other specialised commissioning hubs reflecting the regional and national nature of the post.
The role supports the business in driving improvement, efficiency and transformation in planning, commissioning and delivering services. Post holders will also inform commissioners to help shape future contracting for this patient population. This work will include projected mapping of future service use, identification of users for future service provision, planning in terms of future service needs.
Improving quality and outcomes
- Provide assurance of the quality of the care
- Monitor and oversee service user pathways to ensure high quality services and care planning outcomes are achieved
- Work with providers to achieve national standards and deliver appropriate service/pathway improvements.
- Work with the Network’s and Transformation Lead(s) to understand the existing pathways and review current issues surrounding specialised rehab.
- Transformational projects and pathway improvement.
- Support and lead the development and delivery of plans for service delivery spanning across several organisations (UHS, NHSE, ICB, EHST etc) , identifying risks, issues and dependencies, considering best practice and current options, ultimately making decisions in the best interests of the service. Key Working Relationships
- Operate effectively in a flexible and demanding environment and proactively engage with NHS and independent sector staff, consultants, clinical teams and contractors working on a variety of topics.
- Work effectively with a variety of external consultancies working on other projects and programmes within other NHS directorates as required.
- Provide and receive highly complex, sensitive and contentious information, presenting information, to a wide range of stakeholders in a formal setting.
- Deal with potentially difficult/antagonistic situations as required.
- Work and engage constructively with internal and external stakeholders on a range of business sensitive issues.
- Nurture key relationships and maintain networks internally and externally, including national networks.
- Ensure close liaison with the Communications and Stakeholder team on public relations and marketing activities.
- Link with managers and members of other functions, to address inter- dependencies and ensure alignment.
- Apply a structured change management approach and methodology for the impact of any change.
- Provide cover for other Complex Case Managers, deputising to ensure decision making is progressed to support patient pathways including admission and discharge.
- Work in collaboration with services within Sussex and the South East Region to manage relationships with providers commissioned by other commissioning bodies that provide the referral and aftercare components of the specialised SCI and specialist rehabilitation pathways.
- Work in collaboration with providers and commissioners across a national footprint to ensure patients are provided with the most appropriate care for their needs when this cannot be accommodated in region.
- Work across multiple providers within the NHS and Independent Sector.
- Work in collaboration with local authorities, third sector and other statutory bodies in the management of effective usage of the specialised complex rehabilitation services.
