Working Hours: 35 Hours per week / Full-Time
Contract Type: Permanent
Additional Payments: 10% Flexibility Payment / Essential Car User Designation

This role is subject to -

  • Car Documentation- This post is a designated Essential Car User. Therefore, you must always hold a full valid driving licence and provide your own car for use at work. You will receive the Essential Car User Allowance.

Reablement is a citywide service that supports resident across Manchester. The Complex Care Team provides a life changing service to the people of Manchester by helping them to regain or attain as much independence as they can in their own homes. Staff are fully trained to assist people to meet their care needs, which can range from motivational support to personal care. Complex Care Support Workers do this by motivating and inspiring people to re-learn daily skills and enhance their quality of life.

You will be providing practical support and assistance to individuals with a range of complex needs including physical, sensory or cognitive impairments including promotion of a healthy lifestyle, encouraging and facilitating attendance at a range of appointments and to assist with the intimate personal care needs and all practical aspects of daily life.

About the Candidate

Your duties will include:
  • Providing personal and intimate care to citizens whilst prompting dignity respect and maximising independence.
  • Work sensitively with citizens to build trust in services where misplaced fears may be preventing citizens engage with support around identified needs leading to poor health and wellbeing outcomes.
  • Have a vital role in providing recommendations for the long-term care needs of individual citizen.
  • Co-ordinate citizen’s discharges from hospital and Intermediate Care settings, arranging assessment services to create a support plan and undertake quality assurances checks prior to commencement of service to ensure the required provision of care can be delivered safely for both citizen and staff.
  • Establish citizen’s needs and think innovatively to deliver solutions utilising voluntary sector services and resources to support citizens to reach their goals.
  • Set goals with citizens based on assessed needs develop implement and review support plans.
  • Monitor, evaluate and communicate on the citizen’s progress to the wider multi-disciplinary teams, to establish citizens area of need long term care needed.
  • Engage citizens in healthy living initiatives to adopt and develop sustainable lifestyle changes leading to lifelong healthy behaviours and outcomes.
  • Support citizens in their own home and other locations, enabling them to live with dignity, respect and independence. This includes assisting citizens/ carers with practical household tasks if required.
  • Identify and work closely with community and voluntary groups & partner services to support citizens to build positive support networks, promote inclusion, and meet citizens assessed needs.
  • This role involves shift work that meets the needs of the citizens, these are typically 8.00 to 22:00 hrs and successful applicants will work on a rota that is provided in advance to cover shifts that also include weekends and bank holidays.
