We are looking for community connectors with experience of supporting people with their mental health, with significant experience in statutory and/or voluntary sector services, and a knowledge of Leeds. You will have strong interpersonal skills, be non-judgemental, a good listener and passionate about supporting and empowering individuals to have a better quality of life. You must have integrity and be discreet handling sensitive information.The Community Wellbeing Connectors will support people to achieve better mental health and wellbeing, increasing independence and supporting recovery. They will work with a case load of clients across the Local Care Partnership area, and use a personalised approach to support people, using coaching and motivational interviewing.
Through this they will help clients identify support and services in the local community, that support their recovery, and motivate and enable them to engage with these services. As a community connector, you will become a local expert, gathering and sharing information about local opportunities, activities, and support, bringing people together and supporting them to remain confident and independent in their everyday lives.
