Job Title: Community Engagement Officer

Job Purpose:
To provide one to one support to vulnerable adults with physical disabilities and older people to re-engage with their community where it is identified that they have become isolated. The post holder will work in partnership with the person to determine the barriers they face to Independent Living and establish agreed actions that will remove these barriers.

Job Specific Requirements
- To provide one to one support to some individuals (over a number of sessions) using a strengths-based approach which helps to identify their goals and aspirations and links those individuals to services that can help them to develop independence, self-care techniques, build self-esteem and confidence to reach a point where they can access named services on their own. This will be done by using a range of behavioural change techniques such as motivational interviewing to determine the skills and interests of the individual.
- To undertake functional assessments of a person in a community setting to ascertain the support required to re-engage with their community and/ or prevent further risk.
- To undertake risk, manual handling and trusted assessor assessments to ensure the person is best placed to access the community as independently as possible
- To establish and maintain effective administrative systems in respect of the Day Opportunities team
- To prepare reports in appropriate formats for information and discussion
- To input data and use the Department’s client record system to deal with enquiries, and contribute to the maintenance and accuracy of the information stored in the system by adding and updating as appropriate.
- To liaise with Health, Social Care and 3rd Sector partners to ensure best possible outcomes for a people centred service delivery
- To develop and maintain productive working relationships with all service areas to ensure the highest levels customer satisfaction.
- To undertake training and self-development and to contribute to this process in others
- To Adhere to the Local Authority loan working policy
Person Specification
Job Title: Community Engagement Officer
Education & Training
- A sound working knowledge of using software within a MS Windows (or similar) environment.
- Willingness to undertake all necessary training and development opportunities
- NVQ level 2 Care
- Moving and handling All Wales passport
Experience / Knowledge
- Working directly with members of the public/other agency personnel in a social care, health or advice setting.
- Experience of exploring the potential within local community resources and assisting the person to make and maintain connections
- Experience of working with older people and adults with a physical disability through direct contact
- Experience of liaising effectively with other teams, agencies and opportunity providers to meet anticipated outcomes for the person
- Experience of using IT software.
- Experience of undertaking person centred assessments and designing plans that represent a person and their chosen lifestyle
Skills and Abilities
- Able to communicate effectively using the persons preferred style of communication
- Is positively committed to the success of the team: promoting supportive relationships, sharing information and being sensitive to others.
- Ability to respond sensitively to individuals with physical and personal care needs
- Ability to record information accurately, concisely and factually in a variety of formats
- Ability to work unsupervised
