An exciting opportunity has arisen for a suitably qualified and experienced Registered Nurse to join the District Nursing Service. The applicant must be a Registered Nurse, be confident and competent in a wide range of clinical skills and demonstrate evidence of continuing professional development. There is a requirement for a flexible approach to work and an ability to travel between sites in a timely manner.

There are multiple demands in this exciting role due to the challenging nature of providing care for patients outside of the hospital environment. The post-holder will however find themselves well supported by a very friendly and enthusiastic team of staff.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Cwm Taf Morgannwg University Health Board provides a District Nursing Service that is committed to the principles of holistic nursing care. Underpinning this, the service aims to offer the highest level of clinical excellence and advanced practice with a local community focus. District Nurses play a crucial role within the primary and community healthcare settings and we visit to provide care to patients in their own homes across Cwm Taf Morgannwg. As a nursing service we are committed to working with our patients and also those important to them. We aim to support patients to manage their own conditions and to provide nursing interventions and treatments for those in need, thereby avoiding unnecessary admission or readmission to hospital whenever possible.

It is predicted that the demand for District Nursing services is certain to increase because of a growing elderly population, reduced length of hospital stays and the move to treat more patients, often with complex care needs, in the community. Across Cwm Taf Morgannwg University Health Board, the number of people aged 85 and over is expected to more than double by 2036 alongside increasing numbers of the population living with one or more chronic conditions (WG, 2013).

The Community Staff Nurse is responsible for the delivery of prescribed nursing care to patients on a day to day basis. This would include the holistic assessment of patient care needs and the development, implementation and evaluation of evidence based care. The post holder will be autonomous in his/her clinical practice while acknowledging their limitations and actively seek advice as necessary.
