Bridge View Medical is a merged Practice that operates over four sites- Marksbury Road, Southville, Malago & Gaywood House surgeries. We have c 40,000 patients, cared for by our friendly and professional team of 150 plus dedicated Partners and staff. We deliver high quality healthcare to a diverse population. We are a Primary Care Network in our own right and take an active involvement in the South Bristol Locality.

We have a total of 14 partners with aspirations to continue to build higher employee and patient satisfaction levels supported by a range of clinicians including Paramedics, Pharmacists, Nurse Practitioners, Physiotherapists, Physician Associates and a skilled Nursing Team. They are supported by a dedicated Management team and robust Administrative & Reception teams.

We are looking for an experienced paramedic, with excellent clinical and communication skills, to join our existing team of paramedics and who can work as part of our multidisciplinary team, alongside GPs, advanced clinical practitioners, clinical pharmacists, and other allied healthcare professionals. We expect home visiting to be a significant part of this role and are hoping to develop a dedicated housebound patient service. We see this role as becoming part of this future team and the successful candidate would be able to have input into the design and evolution. You will also be part of the urgent care hub team within the PCN.

You will get 6 weeks annual leave in addition to 8 Bank holidays and 1 week study leave, pro-rata per annum. The role offers flexible working and opportunity to join NHS pension scheme.

Hours of work: 37.5 Hours, Monday – Friday (option to work compressed hours over 4 days)

Salary: £41,500 - £43,807 depending on qualifications & experience

Job Description

The paramedic will be able to work autonomously, using their enhanced clinical assessment and treatment skills, to provide first point of contact for patients presenting with undifferentiated, undiagnosed problems relating to minor illness or injury, abdominal pains, chest pains and headaches. You should be able to demonstrate capability to make sound judgements in the absence of full information and to manage varying degrees of risk when there are complex, competing or ambiguous information or uncertainty.

Key duties and responsibilities

  • Assess and triage patients, including same day triage, and as appropriate provide definitive treatment or make necessary referrals to other clinicians.
  • Advise patients on general healthcare and promote self-management where appropriate, including signposting patients to other community or voluntary services.
  • Be able to: perform specialist health checks and reviews
  • perform and interpret ECGs; alongside other results as appropriate
  • perform investigatory procedures as required
  • undertake the collection of pathological specimens including intravenous blood samples, swabs, etc.
  • perform investigatory procedures needed by patients and those requested by GPs and the wider PCN
  • Support the delivery of anticipatory care plans and lead certain community services (e.g. monitoring blood pressure and diabetes risk of elderly patients living in sheltered housing)
  • Provide home visits and become part of a potential housebound patient service.
  • Communicate proactively and effectively with all colleagues across the multi-disciplinary team, attending and contributing to meetings as required
  • Collaborate with other members of the PCN including doctors, nurses and other AHPs.
  • Maintain accurate and contemporaneous health records appropriate to the consultation, ensuring accurate completion of all necessary documentation associated with patient health care and registration with the practice
  • Prescribe, issue and review medications (if/when qualified) as appropriate following policy, patient group directives, NICE (national) and local clinical guidelines and local care pathways. This will be done in accordance with evidence-based practice and national and practice protocols, and within scope of practice
  • Work with patients in order to support compliance with and adherence to prescribed treatments
  • Provide information and advice on prescribed (if/when qualified) or over-the-counter medication on medication regimens, side-effects and interactions
  • Assess, diagnose, plan, implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis, and patients with complex needs, including mental health and paediatric presentations
  • Clinically examine and assess patient needs from a physiological and psychological perspective, and plan clinical care accordingly
  • Prioritise health problems and intervene appropriately to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care
  • Diagnose and manage both acute and chronic conditions, integrating both drug- and non-drug-based treatment methods into a management plan
  • Support patients to adopt health promotion strategies that promote healthy lifestyles, and apply principles of self-care

  • Enhance own performance through continuous professional development, keep up to date with current evidence-based practice, and impart own knowledge and skills to PCN colleagues to meet the needs of the service
  • Recognise and work within own competence and professional code of conduct as regulated by the HCPC and College of Paramedics
  • Assess effectiveness of care delivery through self and peer review, benchmarking and formal evaluation
  • Participate in research and utilise the audit cycle as a means of evaluating the quality of the work of self and the team, implementing improvements where required
  • In partnership with other clinical teams, collaborate on improving the quality of health care responding to local and national policies and initiatives as appropriate

Administrative requirements

  • Produce accurate, contemporaneous and complete records of patient consultation, consistent with legislation, policies and procedures
  • Be aware of data protection (GDPR) and confidentiality issues particularly within a PCN
  • Use technology and appropriate software as an aid to management in planning, implementation and monitoring of care, presenting and communicating information
  • Review and process data using accurate read codes in order to ensure easy and accurate information retrieval for monitoring and audit processes.

Supervision

The post holder will have access to an overall GP mentor and mentorship programme, as well as overall supervision by an experienced ACP/ANP Lead and a named GP supervisor for each session worked in the PCN to provide support and guidance.

Qualifications

Essential

  • BSc in a training programme approved by the College of Paramedics
  • Health & Care Professions Council (HCPC) registration
  • Able to operate at an advanced level of clinical practice

Desirable

  • Pre-reg MSc in a training programme approved by the College of Paramedics
  • non-medical prescribing qualification
  • Full UK driving license
  • Evidence of verification against the paramedic primary care roadmap
  • Masters / FHEQ Level 7 or SCOF Level 11

Knowledge

Essential

  • experience in managing a designated caseload using theoretical and practical experience, completing assessments, planning, implementing interventions, and evaluating outcomes aligned to care plans
  • ability to write comprehensive clinical notes, implement and evaluate care plans

Desirable

  • mentorship or supervisory skills training
  • be aware of data protection (GDPR) and confidentiality issues particularly within a PCN
