Bradford District Care Foundation Trust (BDCFT) Public Health Nursing, Children's Service are looking for Band 4 Community Nursery Nurses who are innovative, forward thinking, and passionate about making a difference for children, young people and their families.

We are pro-active public health practitioners who work within skill mixed teams, closely with children from 0-19 years and their family, to support them in making healthy lifestyle choices and accessing other specialist services to meet their health needs. The community nursery nurse will provide support to the school nurses and health visitors and undertake reviews, deliver packages of care and facilitate well-baby clinics/ drop-ins, showing commitment in the delivery of the Healthy Child Programme and ensuring our service delivery is of high quality and research based.

Main duties as outlined below will be dependent on the area placed.

Main duties will include:

  • Delivering a public health offer in primary schools/ nurseries.
  • Running and supporting drop-in sessions at the local family hubs/ schools.
  • Offering support/ information and/or delivering packages of care related to various topics, including continence, behaviour, sleep, behaviour, breast feeding, weaning, school readiness, maternal mood and child development and growth.
  • Completing development reviews in line with the Healthy Child Programme, assessing Child development and supporting onward referral to specialist services.
  • Work in conjunction with the school nurse, health visitor or staff nurse where required to assist in meeting the health needs of the child and family.
  • To liaise with the named school nurse/ health visitor regarding complex health needs or safeguarding concerns.
  • Ensure effective communication with the wider team, ensuring any work undertaken is communicated effectively to the named school nurse/ health visitor for the child/family.

You will be solution focused and adaptable to change and able to work both autonomously and part of an integrated skill mix team. You will be able to form good working relationships with other members of the multi-disciplinary team and possess excellent communication skills.

The successful postholder will be positioned into either a school nurse or health visiting team and may be asked to work across both teams.
