An exciting opportunity has become available for a Band 6 Unscheduled Care Practitioner who has the relevant qualifications and experience in Acute Mental Health. The team is based in Llanelli, Carmarthenshire.

The Crisis resolution and home treatment service provides intensive support and treatment for people experiencing mental health related crisis and offers alternatives to in-patient care. The post holder will assess and home treat people who are experiencing mental health crisis. The CRHT works collaboratively with other mental health service providers and is committed to providing care for service users and their families / carers within the least restrictive environment.

Interested individuals will be expected to be enthusiastic, flexible to service needs, and committed to the working ethos of the CRHT model. The Team operates a shift system covering 24 hours, 7 days per week between CRHT and Liaison. The ability to travel between sites in a timely manner is essential.

Applicants will have significant experience working in an acute adult mental health setting and will require a sound knowledge of current issues pertaining to mental health, crisis intervention,home treatment and Liaison.

Comprehensive Assessment, Risk Assessment and Management skills within the context of CTP are essential, as are excellent interpersonal and communication skills.Applicantswill be encouraged to contribute to theteam'sdevelopment ofeducationalprogramme within the general hospitals. The ability to manage time and prioritise workload is fundamental to effective service delivery.

The ability to speak Welsh is desirable for this post; English and/or Welsh speakers are equally welcome to apply.

48 general practices (four of which are managed practices), 47 dental practices (including three orthodontic), 99 community pharmacies, 44 general ophthalmic practices (43 providing Eye Health Examination Wales and 34 low vision services) and 17 domiciliary only providers and 11 health centres;

Numerous locations providing mental health and learning disabilities services;

Highly specialised and tertiary services commissioned by the Welsh Health Specialised Services Committee, a joint committee representing seven health boards across Wales.
