Job overview

Situated in the coastal area of West Cornwall, the Redruth Community Nursing Team cover the locality of Portreath, St Day and surrounding areas and also work flexibly with the other community nursing teams (Penzance, Helston, Camborne, Redruth) when necessary as part of our commitment to providing seamless care.

We are looking for an experienced, caring, motivated and positive community nurse to join our large, busy, professional and dynamic team covering several general practitioner surgeries.

You should possess commitment to team working and innovative delivery of excellent standards of care. You will need to be able to demonstrate a real enthusiasm for community nursing as well as a desire for ongoing professional development.

You should also have an understanding of the changing climate of care delivery in the community.

The ability to travel independently across the West Locality to fulfil the requirements of the post is essential.

10 month fixed term. This is a lone worker post.

The Community Nursing Team is a flexible service, covering 7 days a week, between 8am and 10pm, which includes an on-call service.


Main duties of the job

The purpose of the post is:

  • To ensure provision of a holistic nursing assessment and high quality, safe, evidence based care to patients and carers with the aim of maintaining patients in their own homes.
  • To work in partnership with patients and carers and members of the multi-disciplinary team to offer support and advice and promote informed choice; maximising independence and optimising quality of life.
  • To manage a clinical caseload within a Community Nursing team.
  • To ensure best practice guidelines and performance standards are met.
  • To be an autonomous practitioner responsible for the assessment and treatment of patients with complex health and social care needs.
  • To provide developmental management to all staff within the team.
  • To be responsible for ensuring compliance with Quality and Governance standards, including Health & Safety and patient safety.

Education and Qualifications

Essential criteria
  • NMC Registered Nurse (adult)
  • Evidence of recent relevant professional development
  • Mentorship module or willingness to undertake
  • Relevant First degree or Diploma or equivalent qualification/experience and skills or working towards

Experience

Essential criteria
  • Substantive experience in community nursing
  • The ability to work without direct supervision, yet as part of a team
  • Ability to deal with patients and carers who are under stress
  • Current knowledge of developments in nursing and healthcare in general
  • The ability to provide skilled, evidence based nursing care to a wide range of patients

Skills and Aptitude

Essential criteria
  • IT skills to include use of work, excel spreadsheets, email, internet access, organisational systems
  • Leadership skills
Desirable criteria
  • Advanced computer skills
