Lincolnshire Community Health Services (LCHS) are working in partnership with the acute trust to provide a new and exciting opportunity for any registered qualified practitioner interested in delivering an IV service to patients in their own homes or community settings.
This job is open to all registered practitioners who are able to deliver or train to deliver IV Therapy, including nurses, paramedics both newly qualified and experienced.
A bespoke training package has been developed where you will work closely with members of the IV team as well as the outpatients and day theatre teams
The job will cover 7 days working, full and part time posts will be considered.
This teams aim will be to provide care to the patients within their own homes or community settings to help ease bed capacity within the acute hospitals and to deliver a care closer to home for their patients.
The new adventure is due to start as soon as possible and will involve a small team of registered practitioners working closely together to deliver patients IV Therapy, mainly antibiotics, to the patient within the comfort of their own homes or communities.
Teams are based in three community hospital sites across the county, Gainsborough, Skegness and Spalding.
The post holder will be responsible for the assessment of
person-centred care needs, and lead on the implementation
and evaluation of the OPAT and IV therapy service.
Care will be delivered to a high standard in any health and
care setting that includes the person’s home, the community
or hospital or in any health care setting where their needs
are supported and managed in collaboration with the
multidisciplinary team.

improvements in this area.

To maintain professional registration and revalidation
with the appropriate registered body, at all times, with
professional and clinical knowledge to degree level.
To safely and effectively lead and manage the delivery
of IV therapy, the caseload and team with the
organisation of staff and patient r. Demonstrating
appropriate prioritisation, delegation and assignment of
care responsibilities to others with the organisation of
patient visits, as well as with effective supervision and
evaluation of the team and patient care delivery
To work as an accountable and autonomous practitioner
in the assessment, planning, delivery and evaluation of
safe and effective person-centred nursing care, with
particular focus on IV therapy and prevention of hospital
admissions. Developing and implementing specialist
programmes of care ensuring theory relating to practice.
To be able to communication sensitive, confidential and
complex information concerning the patient’s diagnosis,
treatment and state of well-being this requires empathy
and reassurance. Patients could be hostile at times.
Some patients may have a barrier to their
communication such as English not their first language. To develop, maintain and utilise key clinical skills
required to ensure delivery of the OPAT and IV therapy
service in community and in-patient settings. Working in
partnership and collaboratively with multidisciplinary
team members and statutory and voluntary
organisations in response to individuals’ health, social,
psychological and emotional needs.
Ensure all documentation relating to patient care
acquired by themselves is accurate, legible and safely
stored as per trust policy.
To manage own workload as well as providing day to
day supervision as well as clinical supervision to all staff
or students when necessary.
Undertake surveys and audits as necessary for service,
participating in the implementation of policies and
proposing any changes to practice or procedures within
their work place if necessary
To provide and receive sensitive, complex and
confidential information concerning patients and staff.
Sometimes hostile, and within a highly emotive
atmosphere.
To identify and report any work-related risks in
accordance with Trust policy, utilising the appropriate
reporting mechanisms, co-operating with those with
designated responsibility in the investigation and
management of identified risks.
To take all reasonable care for own health and safety
and that of others, by ensuring equipment is used safely,
is well maintained and in good working order. To ensure
stock control and order as necessary. By attending
mandatory and core training as identified within the
organisation and by adhering to LCHS policies,
procedures and guidelines, which are all available on the
Trust’s website.
To participate in activities to improve the quality,
productivity and effectiveness of care in response to
local and national policies and initiatives. This includes
undertaking any other relevant duties which may be
considered necessary from time to time and which are
applicable to the grade of this post and which are within
the competency of the post holder, following appropriate
consultation and negotiation.
