There is an exciting opportunity to join and established team. Multiple vacancies are available to be part of a team providing care to complex needs children in the community.
Homecare is provision of bespoke care packages commissioned by local Integrated Care Boards (ICB) to provide care for children/young people with complex needs. Homecare is a branch of the Integrated Children’s Community Nursing team (ICCNT) at Alder Hey Children’s NHS Foundation Trust. Carers provide a seamless service and act as an advocate for the child/young person (C/YP) and their family 24/7 Monday to Sunday in their own home or community setting
Develop and maintain clinical competence and skills by undertaking a competency-based training programme, with assessment and re-assessment at regular intervals.
- Provide close observation of the highly dependent child.
- Maintain and meet dietary requirements including enteral feeds according to policy and training.
- Learn clinical skills specific to the child including ventilation and tracheostomy competencies, enteral feeding via gastrostomy or jejunostomy, oxygen therapy, medication administration, oral – nasopharyngeal suctioning, saturation observations, and basic observations.
- Undertake interventions e.g., physiotherapy
- To carry out all aspects of care according to set guidelines, policies and procedures after completion of a competency-based training programme.
- Maintain written clinical records using Trust documentation.
- Work in partnership with the multidisciplinary team according to the care plan under the guidance and delegation of appropriate professionals.
- Communicate effectively with the child and family acting as an advocate when needed.
- Communicate efficiently with all professionals to provide a seamless service
- To work in conjunction with the family, teachers, other carers, long term ventilation team and other health care professionals to ensure a holistic approach to the child’s care, wherever it is needed, and working without direct supervision.
