JOB SUMMARY
To provide the highest standard of care to all Service Users in their own homes. To sustain, support and enhance their independence.
DUTIES & RESPONSIBILITIES
- Assisting all Service Users in carrying out self-care tasks, which will cover washing, showering, bathing and bed bathing, dressing and undressing, toileting and feeding
- Assisting with continence management, catheter care, stoma care, all in line with the Service User care plan
- Carrying out housekeeping duties including tidying up rooms, making up beds, disposal of incontinence products, emptying and cleaning of commodes
- Assisting with fire lighting, shopping, cooking, dish washing and laundry
- Providing a day sitting and night sitting service to Service Users
- Providing Service Users with refreshments and light snacks as appropriate and assisting with feeding when required
- Managing medication, when this is approved and detailed within the care plan
- Cleaning and maintaining spectacles, false teeth and hearing aids
- Being particularly vigilant for signs of distress or anxiety in Service Users; any deterioration in physical and/or mental health, safety of their environment and report and concerns as a matter of urgency to your Area Service Manager (ASM)
- Upholding Service User rights, maintaining confidentiality at all times and preserving dignity
- Maintaining regular communication with the ASM and informing of any changes to Service User health or needs
- Notifying the office of any changes to rota times
- Ensuring when access is not granted to a Service User home the office is telephoned immediately and a call card is left
- Ensuring the care provided in the community is to all the required standards and all codes of conduct and practice are observed from all of appropriate regulatory bodies
- Maintain good relationships with other external agencies involved in the care of the Service User, i.e. community nurses, occupational therapists, etc
- Participate in reviews of Service User care plans, as required
OTHER DUTIES
- The Community Care Worker must maintain good quality written records I line with company policies and procedures
- Provide cover for colleagues on annual leave or any other absence
- Notify the ASM or emergency on-call phone of non-attendance
ESSENTIAL
- Full driving licence and access to a motor vehicle
- Great communication skills
- Able to promote and deliver person centered care
- Able to promote dignity and respect when delivering care
- Be well organised/professional/caring
- Interested in working in the community within Health & Social Care
- Care about the community they live in
