An opportunity has arisen for you to join us working on a part-time 12 month fixed term contract to cover maternity leave leave.
As a Community Care Worker you will undertake holistic needs-led assessments in partnership with service users, patients and carers, prepare care plans, commission services and manage a caseload to ensure the most effective and beneficial care packages are established, delivered, monitored and reviewed.
- Assessments are undertaken within a multi-disciplinary setting, and reflect the perspective of the individual and their carer, and lead to care plans which promote maximum independence and promote outcomes which are agreed with the individual who is being assessed
- To be responsible for managing and prioritising a high volume personal caseload of varying complexity
- To maintain accurate and up to date case records, and timely completion of care management forms as required by data management systems (PARIS).
- To take notes at case conferences and strategy meetings for vulnerable adults where an Administrator is unavailable and where the post holder is the care manager.
- To act as a Duty Officer dealing with emergencies and same day urgent assessments as required.
- To undertake holistic social care assessments, in partnership with service users, families and carers, prepare care plans, commission services to ensure the most beneficial and cost-effective care packages are established, delivered, monitored and reviewed, and comply with all statutory requirements for Torbay and South Devon NHS Foundation Trust policies and guidelines.
- To have clear responsibilities for the safeguarding of adults and children and for operating in a manner consistent with Human Rights legislation
