An exciting opportunity has arisen enabling the recruitment of a Community Care Worker to join our South locality team. The worker will carry out social care assessments and support customers to create individualised support plans to meet their identified needs. The successful candidate will be working with a wide range of customers aged 18 and over including individuals with Learning Disabilities, Physical Disabilities and Older adults.

You will need to have excellent communication and listening skills, with the confidence and ability to negotiate and challenge appropriately. You will need to have an understanding of the Care Act 2014 and be able to implement a strengths-based approach to your work with customers, their carers and support networks. You will be able to manage a case load independently, and work as part of a supportive team.

All employees will normally be appointed to the minimum of the grade for the job. For reasons connected with a particular need of the Council it may be appropriate, subject to approval, to appoint to a higher point in the pay grade.
