An exciting opportunity has arisen to join our Community Mental Health Service working specifically within the Social Care Mental Health Team.
We are looking to appoint a Community Care Worker with good assessment skills and a demonstrable commitment to placing customers and carers at the heart of the assessment and cost-effective support planning - applying a strength’s-based approach.
You should ideally have experience of working in social care with adults with severe mental ill health/dementia and multidisciplinary working. The successful candidate will have excellent communication and organisational skills and a good understanding of working with Adults and Carers with a commitment to safeguarding and improving outcomes for Adults. The role will focus on completing robust reviews of existing support packages and arrangements and also supporting safe and effective discharges from hospital.
The role will also involve working as part of a duty team supporting the duty Social Worker on a rotational basis and you will therefore need to work well to complete tasks in a timely and effective manner.
Applicants should already hold a NVQ or equivalent in a relevant field. You will be required to travel around various locations and have your own transport available.
The successful candidate will be part of the Specialist Mental Health Service and will also be required to work closely with partner agencies across the Public, Private, voluntary and Independent Sectors.
