The Role

Your role will be to support people through an assessment process (Person Led Assessment) to identify their needs, focusing on their strengths. This will involve seeking and encouraging the contribution of people, their carers and relatives as well as others involved in their care, including colleagues from mental health integrated teams.

This assessment will form part of your role in ensuring people are supported to meet their agreed outcomes through commissioning support in conjunction with the Support Options service. Once this support is in place, you will review this to make sure it is meeting people’s needs and outcomes as expected.

Working in Social Care is a challenging and rewarding job. With your help people can remain independent for longer and have choice and control. In Kirklees we aim to deliver high quality, personalised services.

To be considered for this role you will need to demonstrate the following skills, abilities and experience on your application form:

  • Experience of working in a caring environment in a paid or unpaid capacity with people with mental health needs.
  • Ability to identify and assess social care needs of individuals and formulate support plans.
  • An understanding of procedures relating to the relevant service user group.
  • Ability to demonstrate literacy, numeracy and computer skills to complete assessments and correspondence, complete financial assessments and keep accurate records.
  • Ability to prioritise and meet deadlines.
  • Understanding of other agencies and their contribution to service delivery.
  • Willingness to continue further personal training and development and to agree a personal development plan.
  • Possession of a full and valid driving licence (exceptions can be made for disabled applicants). Travel is an essential part of this job and therefore it would be extremely difficult for you to do your job effectively if you did not use a car for all or most of the time.

What can we offer?

  • Generous annual leave entitlement
  • Friendly and supportive team environment
  • Ongoing training and career development
  • Excellent pension scheme
  • A range of family friendly policies, including flexible working and working from home
  • We offer a wide range of well-being initiatives including a cycle to work scheme and discounted KAL membership

This job is subject to a DBS check at the appropriate level, please refer to the Job Description/Profile. A conviction may not exclude candidates from appointment but will be considered as part of the recruitment process.
We know there’s a wealth of talent among people who have a disability and we encourage applications from people with all differing abilities. So, if you need any support completing an application form, or any other format for the application or Job Description please contact the Recruitment Team for help by email: [email protected] or phone: 01484 221000 and ask for ‘Recruitment’.
We are committed to safeguarding and promoting the welfare of vulnerable adults and children and young people and expect all staff and volunteers to share this commitment.
