Job description
We have a great opportunity to recruit into our specialist leadership team.
We are looking for a creative, passionate & motivated registered professional to come & take our Community Rehab team to the next stages in its development.
Hilda Johnson House is a unique 8 bed supported living unit supported by the community rehabilitation team. Staff are based within the unit but their primary aim is to support individuals with enduring SMI in the community; to reach their optimum level of recovery, work towards engagement with CMHT’s & prevent relapse.
Candidates should have experience in leading a team to think differently, engaging with external partners & working in a collaborative manner.
This role combines leadership with clinical input. Candidates will need to be able to work flexibly & be responsive to the complexities of the service.
The community MDT consists of RNs, STR workers, HCSW’s, Occupational therapist & assistant & Psychologist and will work closely with Inpatient rehabilitation Services & our Complex Care Team to ensure there is a seamless Rehabilitation Pathway for our service users.
Candidates will be asked to prepare a 10 minute presentation on: “The Future of Rehabilitation, your 5 year plan for the service”.

Provide management and clinical leadership to the multi-disciplinary team to ensure care is delivered which is person centred and meets standards for Quality and Safety, in partnership with service users and carers.
The post holder will lead from the front & support the team to reach its full potential. Supporting all members of the MDT to develop & ensuring service users receive the highest standards of care during their journey through the service.
The post holder will be a key team member in the wider rehabilitation MDT, working closely with colleagues from our inpatient rehabilitation team, CMHT’s & external providers. Building new relationships to ensure our service users have the best access to community support & opportunities, both pre & post discharge from the service.
The role will include oversight of the 8 bedded facility at Hilda Johnson House.

Management and clinical responsibility for the Team, ensuring processes are in place for the delivery of safe, compassionate care in accordance with Trust policies and procedures.
To provide strong clinical leadership as well as work as a member of the multi-disciplinary team.
Lead a multi-disciplinary team of staff in the delivery of high quality evidence based practice, ensuring that regular supervision and appraisal takes place.
Ensure systems are in place to manage administrative and operational resources in a cost-effective manner, evaluating the standard of service delivered.
Co-ordinate multidisciplinary working, maintaining excellent communication and working relationships with all team members, to ensure effective interface and continuity of care between disciplines and services.
Role model best practice, where appropriate undertaking clinical procedures to a standard that ensures safe and effective care, and complies with the relevant Code of Conduct or relevant professional bodies.
Management of clinical care within the team, ensuring that clinical pathways and Trust values underpin the practice and delivery of care on the unit.
To work within and across professional and organisational boundaries to deliver integrated, individualised and expert care and leading communication with all stakeholders.
The post holder will work alongside and deputise for the Service Manager when required. To support with the day to day management, quality and performance of the service.
Demonstrate highly developed clinical skills and act as clinical advisor and educator to junior staff.
To provide support and guidance to colleagues over issues of risks and / or complex clinical presentation.
