Job description
The Homeward Assessment Team (HAT) is a joint collaboration of the Urgent Care Directorate, GHC and Frailty Service at GHNHSFT. Based at the Emergency Department, Gloucester Royal Hospital. The teams purpose is to provide a rapid ( within 15 minutes of referral ) assessment of patients to prevent admission to the acute trust.
The HAT team are extremely supportive. New staff are allocated a mentor who will support and guide individual development.
There is an approx 6-8 week induction period supernumerary to the off duty. This allows for any visit to other teams/services/completion of competencies to be achieved.
A good sense of humour is essential.
- To assess a cohort of patients within the Emergency Department (ED) from presentation through to discharge.
- Be proactive working closely with the GHNHSFT Frailty Service.
- Through training and development educate GHNHSFT and staff of available discharge pathways and signpost to enable discharges out of service hours.
- Working with colleagues, to risk assesses suitability for discharge and ensure that arrangements are appropriate and timely.
- To communicate information on discharge to inter-disciplinary teams, multi-agency teams, outside providers, patients and carers.
- To work with community services, ie CATU, GHC Rapid Response Team (RR) to help develop a system of ‘pulling’ patients’ - higher acuity patients out of the acute hospitals.
