Job description
- Mentor and coach pharmacy foundation pharmacists, pharmacy students, pharmacy technicians and Pre-registration pharmacy technicians. • Support practice staff, clinical and non-clinical with training on prescribing issues. • Work closely with GPs to resolve day to day medicines issues. • Work with patients to self-manage their well-being and long term conditions, through optimising medicines.
- Manage the repeat prescribing reauthorisation process by reviewing medication requests, medicines reaching review dates and medicine queries. This may include prescribing where necessary. • The independent and autonomous assessment of undiagnosed illnesses in patients presenting to a surgery as they would to a regular GP. • The assessment of the patient including onward referral as well as review and follow up.
- Co-ordinate clinics & review pathology results for patients taking high risk medicines e.g. methotrexate, lithium. Action results as per local shared care arrangements. • Run clinics for newly registered patients, to ensure all repeat medications are prescribed, chronic disease monitoring is undertaken and patients are triaged as appropriate.
- Support reception staff to deal with medicine related queries and phone calls. • Extended hours sessions • To reconcile medicines at interfaces of care, most commonly discharge from hospital. To include, identifying and rectifying any unexplained changes, completing clinical medication review and arranging follow up such as follow up tests, dose titration and medication supply where appropriate. • Reconcile medication records and arrange medication supplies for patients following recommendations from a secondary care outpatient appointment.
- Prescribe independently within competence, where appropriate. • Contribute to reducing medicines related hospital admissions and readmissions by proactively targeting reviews at patients identified as being at greatest risk. Specifically to include frail / elderly in Care Homes and in receipt of domiciliary care. • Support the GPs and other practice staff to deliver QOF and enhanced services.
- Conduct clinical audit as part of the multidisciplinary team and ensure implementation of any agreed actions. • Lead the practice on implementing quality improvement in prescribing e.g. devise improvements to repeat prescribing systems, improve patient safety or improved adherence to NICE recommendations. • Working with the practice team develop evidence based practice prescribing guidelines and formularies • Contribute to clinical education of other healthcare professionals.
- Lead clinical service developments and improvements related to implementation of evidence based medicine and prescribing processes. • Lead on implementation of drug alerts e.g. MRHA and drug withdrawal / supply issues affecting patients.
