We have an exciting opportunity for a Clinical Pharmacist to join our Primary Care Network based in Plymouth. A key member of the practice responsible for medicines management services across the organisation. The post holder will work as part of a multidisciplinary team to manage repeat prescription authorisations and reauthorisation, acute prescription requests, management of medicines on transfer of care and systems for safer prescribing.

We are a GP single-practice Primary Care Network based within the Plymouth area, providing care to over 30,000 patients. We aspire to delivering excellence in health care and feel the best model to achieve this is one where we have a unified service working across multiple locations including Plympton, Laira, Efford, Crownhill and the city centre. This gives our patients choice: choice over where they visit and who they see. This is particularly useful for patients that need a little extra expertise – many of our clinicians have a specific area of interest and sometimes it is worth travelling to see them for their extra skills in a specific area.

Role Responsibilities:
Medication Review:
- To undertake medication reviews with patients and propose changes as appropriate.
- Reduce inappropriate poly-pharmacy and wasteful prescribing through medication review.
- Support medication reviews of care home residents in line with current guidelines.
- Support medication reviews of vulnerable housebound patients at risk of hospital admission and harm from poor use of medications.
- Management of medicines at discharge from hospital.
- Reconcile medicines following hospital discharge and intermediate care. Identify and rectify unexplained changes, manage these changes, perform clinical medication reviews and produce post discharge medicines care plan. Work with patients and community pharmacists to ensure patients receive the medicines they need post discharge.
- Contribute to reductions in medicine related hospital admissions and re-admissions by supporting patients to get the best outcomes from their medicines and identifying and addressing medicine related issues.
- Work with hospital colleagues to proactively manage patient pre-discharge (e.g. care of the elderly, doctors and clinical pharmacists) to proactively manage patients at high risk of medicine-related problems before they are discharged to ensure continuity of care.
- Implement practice searches of patients most likely at risk of unplanned admission and re-admissions from medicines.
Repeat Prescribing:
- Manage the repeat prescribing re-authorisation process by reviewing requests for repeat prescriptions and medicines reaching review dates; make necessary changes as an independent prescriber, and ensure patients are booked in for necessary monitoring tests where required.
- Review daily pathology results for patients on known medicines.
- Manage practice formularies to improve the quality, safety and cost effectiveness of prescribing.
Medicines Information:
- Manage patients and health care professionals’ medicine queries.
- Act as a source of medicines information for all of the practice team and patients (e.g. around doses, side effects, adverse events, possible alternatives e.g. low/out of stock.
- Provide medicines information and training to Practice healthcare professionals and admin staff.
- Work with community pharmacy to promote and refer patients in to the community pharmacy minor ailments service: dealing with minor ailments and triaging patients appropriately.
Medication Safety and Quality:
- Increase quality and safety of prescribing through mechanisms such as audit and PDSA cycles.
- Implement drug withdrawals and alerts e.g. MHRA aimed at improving medicines safety.
- Work with primary care professional and patients to implement NICE and other evidence based guidelines.
- Provide leadership to the Practice Manager and GP’s to ensure the practice is compliant with CQC standards where medicines are involved.
The ideal candidate will have the following skills:

- Experience of autonomous practice working and multi-professional team working.
- Ability to work on own initiative and prioritise task effectively.
- Ability to make clear decisions with confidence and communicate these effectively.
- Ability to work under pressure while remaining calm in difficult situations.
- Ability to effectively assess, plan, treat and refer patients appropriately and evaluate patient care confidently
- Broad and sound knowledge of complex and common conditions in primary care in both adults and children.
- Experience of working to protocols / guidelines and patient groups / patient specific directives.
- Professionalism and discretion when dealing with sensitive information, maintaining confidentiality at all times.
The ideal candidate will have the following qualifications and experience:
- Completion of an undergraduate degree in Pharmacy, and registration with the General Pharmaceutical Council.
- Minimum of 1 years post graduate experience in pharmacy, as demonstrated within a practice portfolio.
- Member of the Royal Pharmaceutical Society (RPS).
- Must hold or be working towards a postgraduate pharmacy qualification.
