Leeds Student Medical Practice and The Light Surgery Primary Care Network (LSMP & TLS PCN) are looking to recruit an enthusiastic and forward-thinking Clinical Pharmacist. We want the most resilient people to join our growing PCN team.
The role will involve working alongside our Lead GP and the Pharmacy Team, which currently consists of one full time PCN Senior Clinical Pharmacist, a PCN Clinical Pharmacists and a Pharmacy Technician. This will be an exciting opportunity for the successful candidate to join our team and help influence and shape the future direction of the team. Direct patient care is a large part of this role and patient reviews will be carried out alongside the team of PCN Clinical Pharmacists. It is desirable for the successful candidate to have experience of service development and experience in working in Primary Care, however this is not essential.
The PCN comprises of two Practices; both are committed to creating a supportive working environment where the role will develop over time. The successful candidate will be supported to complete the CPPE pathway with dedicated training and support from GPs and the Senior Clinical Pharmacist.
The Clinical Pharmacist should be able to work autonomously and be able to provide support to our patients to manage their long-term conditions effectively, through complex polypharmacy medication reviews. In addition to playing a pivotal role in our vaccination programmes.
The patient population across the PCN is predominantly students and young working professionals; therefore, it is essential that the successful candidate will be able to demonstrate interest in helping young people manage their medication and also have a proactive view on disease prevention. Experience or an interest in Mental Health management and prescribing is desirable as the patient population based across the PCN has a large percentage of mental health patients.
The successful candidate should be able to provide advice and guidance to practice staff and patients with regards to prescription and medication queries, policies, procedures, IIF, QOF, local incentive schemes, audits and medicines safety.
Our PCN does not have any care homes attached to our footprint.
