Job description
The postholder will: Co-ordinate and participate in the provision of a full range of lung cancer specialist services Provide specialist holistic assessment and evaluation of individual patients needs referring to other multidisciplinary team members, as appropriate. Action plans to be developed. Participate in the training and assessment of staff in respect of lung cancer care Liaise and communicate effectively with all members of the multidisciplinary teams, attending the MDT meetings and contribute to the discussions and patient assessment / care planning decision at the weekly meetings if appropriate. Ensure a high quality of written and other information is available for patients and their carers Provide specialist advice and information to professional colleagues, both in primary and secondary care Participate in the development of nurse led clinics as appropriate, and evaluate their effectiveness Evaluate the patient pathway and develop referral and discharge criteria for specialist intervention Ensure written consent for clinical procedures Regularly contribute to clinical decisions regarding changes to patient care interventions Be aware of appropriate Trust policies and procedures In collaboration with the Lung Cancer Clinical Nurse Specialist, develop, implement and monitor quality assurance and risk management initiatives within the clinical area in line with the Trusts strategy for quality and risk management Lead on patient communication issues and co-ordination of the patients pathway for patients referred to the team acting as the key worker or responsible for nominating the key worker for the patients accessing with the team.
