Job summary
Local Care Direct is a non-profit Social Enterprise operating across 24 hours a day, 7 days a week, 365 days a year, to provide a range of primary and urgent healthcare services across West Yorkshire supporting the Local Community.
We are looking for an enthusiastic, experienced and knowledgeable Patient Safety / Clinical Governance professional to join our Corporate Clinical Team part-time working 22.5 hours per week.
The Clinical Governance & Patient Safety Practitioner would have a main base at Lexicon House in Leeds but would work across all Local Care Direct sites as required.

Main duties of the job
The Clinical Governance & Patient Safety Practitioner will support the Clinical Governance, Quality and Patient Safety function within Local Care Direct.
This will include implementing the patient safety strategy and clinical governance standards across Local Care Direct. Along with working closely with the organisations patient safety partners.

Job Title: Clinical Governance and Patient Safety Practitioner

DBS Check Level Required: Enhanced with child and adult workforce & barred checks
Hours of Work: 22.5 hours per week
Job Purpose:
- The purpose of this position is to support the Clinical Governance, Quality and Patient Safety function within the organisation. This includes implementing the patient safety strategy and clinical governance standards across Local Care Direct.
KEY PRINCIPLES
- This is a clinical post and the post holder must ensure they maintain any professional and regulatory standards at all times.
- Deputise for Clinical Governance & Quality Lead| Patient Safety Specialist.
- To support the delivery of the Patient Safety Strategy across the organisation.
- To support the delivery of the Clinical Governance Function.
- The application of clinical governance principles to the delivery the service. This includes the use of policies and standard operating procedures; reporting, investigating and learning from adverse incidents, complaints, concerns, patient experience and clinical audit.
- To act in an advisory and supportive capacity to the Directors and managers of the organisation

- To be responsible for the implementation and monitoring of patient safety training across the organisation
- To support the development and implementation of the Patient Safety and Incident Response Framework and associated plan.

Specific requirements to meet the Key Deliverables
Patient Safety Strategy
- To support the Patient Safety Specialist with the implementation of the Patient Safety Strategy across LCD.
- Promote patient safety understanding across the organisation.
- Participation in improvement activity to embed systems thinking, human factors understanding and just culture principles in all patient safety processes
- Lead and participate in patient safety investigations, providing support to colleagues/ families as necessary
- Promotion of Just Culture across the organisation
- Provide patient safety expertise/leadership within the organisation.
- Support the effective collation, analysis and presentation of qualitative and quantitative patient safety data and provide regular and tailored reports to all relevant committees
- Support the development and implementation of continuous improvement of quality and impact of incident investigations, through the new Patient Safety Incident Response Framework (PSIRF)
- Support clinical teams in managing and providing safe services through risk management of environmental, clinical and organisational risks to patient safety. Working with staff to collate timely and robust insights to support patient safety improvement.
- Lead on recruitment and development of the organisation Patient Safety Partners.
Clinical Governance
- To undertake clinical audits as required, using appropriate research methodology to benchmark, reporting to the relevant committees and groups
- As a member of the Clinical team the post holder will attend clinical meetings as agreed with the Director of Clinical Governance & Quality
- The post holder will work closely with colleagues across the organisation to ensure that clinical governance is fully embedded.
- To provide and receive highly complex or highly sensitive, contentious information, being aware that there may be occasions when there will be significant barriers to understanding.
- To prepare reports and analysis related to investigations to external bodies including CQC, HCPC, NMC, GMC and other professional and legislative bodies, e.g. Coroners.
- The post holder will be required to participate in relevant development activities and development reviews.
- To lead on the resolution of general Clinical Governance matters as they arise.

- To operate and ensure the team operates within information governance protocols and that business confidentiality is maintained at all times.
Workforce Development
- The post holder will contribute workforce planning and development as required to support the Clinical Governance function
Project management
- The post holder will be responsible for the delivery of key projects as identified and agreed with the Clinical Governance Lead| Patient Safety Specialist/ Director of Clinical Governance and Quality.
- Project plans will be prepared and managed in line with project management principles. Update reports will be provided as agreed with Clinical Governance Lead| Patient Safety Specialist/ Director of Clinical Governance and Quality.
Communications & Relationships
- To promote effective cross-functional working and that the operational team supports the achievement of objectives, standards and protocols of other LCD areas.
- To attend internal and external meetings representing LCD and engaging in discussions appropriate to the role and level of the post.
- To provide regular reports on the performance and development of the team at 121s and internally and externally as required.
Patient Care
- To ensure that the quality of patient care is firmly at the centre of the organisations objectives and vision.
- To ensure the management and monitoring of the highest standards or safety and quality in the delivery of healthcare and related services, and manage risk within Local Care Direct.
Financial Resources
- To ensure financial policies and procedures are maintained and the dental budget is adhered to and that best value for money is achieved.
- To prepare business cases as required and agreed with the Chief Operating Officer.

- To operate and ensure the team operates within information governance protocols and that business confidentiality is maintained at all times.
Training
- To complete the defined company induction programme and core training for this role and mandatory refresher training thereafter and ensure that accurate records of training are maintained.
- To ensure all staff receive an annual performance and development review incorporating the development of a personal development plan.
- To seek feedback on their performance.
- The post holder will support and deliver training as required by role to support the clinical governance function.
Infection Prevention & Control
- Healthcare workers have a duty of care to patients and are expected to comply fully with the companies Infection Prevention and Control (IP&C) policies and attend/complete IP&C training, for personal and patient safety and for prevention of healthcare-associated infection (HCAI). Knowledge, skills and behaviour in the workplace should reflect this.
