Job description
The post holder is a member of the Clinical Coding Team who translate medical information (diagnoses and interventions) from patient’s electronic record to assign appropriate clinical codes using ICD-10 and OPCS-4 classifications. This information must be timely, complete and accurately recorded. It is required for cost analysis, invoicing CCGs, treatment effectiveness and outcome measurements.
- Undertake the Clinical Coding Standards Foundation Course (25 days) and pre-course elearning modules. Course content includes Anatomy & Physiology, Classifications of Diseases, History and theory of Clinical Coding through to assigning clinical codes in a classroom situation.
- Demonstrate by assessment an understanding of the rules and conventions that surround the Classifications of Diseases and Healthcare Related problems, tenth revision (ICD-10) and Office of Population Census and Surveys fourth revision (OPCS-4) at the end of the foundation course.
- Identify in the patient medical record, what clinical information needs to be collected and to correctly assign clinical codes to diagnoses and procedures in line with current national standards.
- Maintain of a good level of competency in the recording of clinical codes. Complete assessments as and when required to measure knowledge and accuracy of clinical coding.
- Attend dedicated clinical coding computer training sessions which will enable to be able to navigate the MEDICODE software successfully and gain full use of this system.
- Undertake local and national clinical coding training programs held either internally or externally as required and work towards the National Clinical Coding Qualification (NCCQ).
- Complete clinical coding to the highest level of specificity using the three dimensions to coding accuracy giving complete, accurate data of a high quality that is consistently recorded.
- Ensure all coding is completed in a timely fashion in line with Trust and billing requirements. All clinical coding has to be completed by a given date in the month following the month in which the patient is discharged or transferred to another specialty or consultant.
- Report to the Clinical Coding Team Leader any discrepancies in clinical information contained in the medical record that contradicts clinical coding guidelines.
- Promote and adhere to clinical coding policies and procedures and national guidelines as laid down in the Data Quality Review and the National Classifications Service. Maintain these documents as and when new guidelines are issued.
- • Carry the on-call Medical Records Major Incident bleep as per rota. The week on call is for 24 hours, 7 days a week. This would entail being available at short notice i.e maximum of 30 minutes distance from the hospital, in the event of a Major Incident.
