The post holder will play a key role in supporting the Personalised Healthcare Commissioning service across the region. Working within the remit of the National Framework for Continuing Healthcare and other NHS funding streams, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.
The post holder will be responsible for assessing patients, ensuring The National Framework for NHS Continuing Healthcare Funded-Nursing Care and S117 entitlement is implemented and complied with, according to the principles and processes.
The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.

The postholder will be working within a team of Personalised Healthcare Assessors, taking a lead role in complex cases, ensuring a high-quality service is delivered, supporting and mentoring staff to achieve continuous improvement.
As a Senior Practitioner, this would include a caseload of complex cases and management of Personalised Healthcare packages of care, ensuring value for money placements which meet the needs of the individual and identify opportunities for development and improvement to assist delivery of best practice healthcare.
The successful candidate will be expected to manage a caseload of patients with complex needs, contributing to performance improvement and taking a lead for identified areas where agreed
Other duties will include managing staff in line with regional and local HR policies, being involved in recruitment, supporting and mentoring of junior members of staff, facilitating and attending MDT meetings and assessments, supporting the PHC team , ensuring all processes implemented are compliant with the NHS Continuing Healthcare National Framework and contributing to the overall quality and performance of the team.
You will be expected to assist the PHC Team Leader with the resolution of complaints, appeals and disputes and deputise for the PHC Team Leader, during their absence

- Managing a caseload of patients and contributing to performance improvement, taking a lead for identified areas, where agreed
- Managing a defined caseload for those with complex health needs
- Managing staff in line with regional and local HR policies
- Facilitating and contributing to multi-disciplinary assessments and patient reviews in line with standard operating procedures
- Active involvement in the recruitment of junior members of staff
- Supporting the Personalised Healthcare team to ensure that workstream plans are fulfilled and supervised effectively to ensure successful delivery
- Supporting the Team Leader to ensure that all processes implemented are complaint with the NHS Continuing Healthcare National Framework
- Deputise as required in the absence of a team leader for Personalised Healthcare Commissioning
- Assisting the Team Leader for Personalised Healthcare with resolution of complaints, appeals and disputes
- Raising issues of concern through the Senior Management Team where necessary
- Mentoring and supervision of Personalised Healthcare Associate Practitioners, Practitioners and students
- Contributing to performance improvement within the team of Senior Practitioners, Practitioners and Associate Practitioners ensuring that services are delivered to a high standard
- Monitoring workstreams of Personalised Healthcare Practitioners and quality assurance of processes
- Working with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions
- Actively supporting the Personalised Healthcare team in working towards goals and objectives
- Communicating information, risks, issues and dependencies, including briefings and reports
- Maintaining professional registration and up-to date knowledge of legislation
- Providing relevant and timely specialist advice and guidance on functional and information matters relating to the service
- Delivering against objectives, achieving quality outcomes, prioritising own workload and working to tight deadlines
- Fostering transparency and a person-centred approach with patients, families and representatives
- Promotion of the use of Personal Health Budgets for eligible individuals as a means of their having control over their care and achieving best possible outcomes whilst being cost effective and value for money
- Ensure the principles and duties of safeguarding are holistically, consistently and contentiously applied
- Contribute to maintenance and monitoring of service standards around quality
- Comply with NHS Codes of Conduct and Standard Business Conduct and/or NHS Trust Standards of Business Conduct policies
