CHC Clinical Specialist Nurse (Band 7)
Eden Brown Synergy are actively recruiting for an experienced CHC Clinical Specialist Nurse for an interim opportunity we currently have available on a hybrid basis.
Our client are looking for someone to manage a team of Continuing Healthcare Nurse Practitioners responsible for facilitating multi-disciplinary assessments and patient reviews whilst ensuring that their team adheres to the relevant National Framework and guidance and the NMC Code of Professional Conduct to make sure that quality standards and performance are always met.
Key Responsibilities:
- Support the Clinical Manager to ensure that their work-stream is planned and managed effectively and to take the lead in assisting in their successful delivery.
- Oversee the delegation of workload within the team.
- Managed a defined caseload for those with complex health needs.
- Work with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions.
- To ensure correct Continuing Healthcare guidance and process is followed and that services are delivered to maximise patient safety.
- To provide cover for other Specialist Nurses as appropriate.
- Ensure the clinical team are maintaining data systems in line with standard operating policy and procedures.
- Provides relevant and timely specialist advice and guidance on functional and information matters relating to the service.
To be successful in this role, you will need:
Educated to degree level in relevant subject or equivalent level qualification or significant experience of working at a similar level in specialist area, ideally CHC.
Registered Nursing Qualification and current registration with NMC.
Evidence of post qualifying and continuing professional development.
First line management qualification or experience or evidence of working at this level.
Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
Demonstrated capability to act upon incomplete information, using experience to make inferences and decision making
