Following a re-structure of our ways of working a fantastic opportunity has arisen for proficient and highly motivated registered clinicians (Nurse or AHP) to join our existing Continuing Healthcare team. This role require a dynamic individual who is an effective communicator, is forward thinking and able to perform both independently and as part of a team.
The ideal candidate will have the ability to co-ordinate and manage competing priorities, good organisational skills, time management and a solid understanding of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care.
Your daily working environment will offer varied and interesting aspects of NHS services. Continuing Healthcare is an assessed funding stream and therefore you will get the chance to interact with various stake holders, patients, and their families on a regular basis. If you are able to work proficiently, flexibly, accurately and able to meet the challenge then this is the job for you.

BSW ICB have adopted agile working so this means the successful applicant will predominately be working from home but may be requested to work in any one of the ICB’s office bases across BaNES, Swindon and Wiltshire. You will also have a contractual base within the BSW ICB office where your team is based

The post holder will lead in the implementation and delivery of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care (revised 2012) and be able to apply it within the context of assessment, review and identifying outcomes for the assessed needs of individuals, which will include assessing individuals with mental health needs and or dementia.
