Job Summary
We are seeking a self-employed Catastrophic Case Manager with a background in social care, particularly in brain injury rehabilitation units and care homes. As a self-employed Case Manager, you will play a vital role in coordinating and overseeing the care and support provided to individuals with catastrophic injuries. Your expertise in brain injury rehabilitation and care home environments will be essential in ensuring the well-being and recovery of our clients.
Responsibilities:
Case Management:
- Conduct comprehensive assessments of clients' social and care needs.
- Develop individualized care plans and coordinate services to meet clients' specific requirements.
- Monitor and evaluate the quality and effectiveness of care and make necessary adjustments as needed.
- Act as a liaison between the client, their family, and the various healthcare professionals involved in their care.
Rehabilitation Planning:
- Collaborate with interdisciplinary teams to create and implement rehabilitation plans tailored to clients' goals and abilities.
- Coordinate and monitor the delivery of therapeutic interventions, including physiotherapy, occupational therapy, speech therapy, and psychological support.
- Advocate for appropriate resources and equipment to facilitate clients' rehabilitation and maximize their independence.
Care Coordination:
- Liaise with care homes and brain injury rehabilitation units to ensure smooth transitions and continuity of care for clients.
- Provide guidance and support to care staff in managing the specific needs of individuals with catastrophic injuries.
- Collaborate with community resources and support services to facilitate clients' integration into the community and enhance their quality of life.
Family Support and Education:
- Provide emotional support and education to clients' families, helping them understand the implications of catastrophic injuries and the available support services.
- Assist families in navigating the healthcare system, accessing financial resources, and advocating for their loved one's needs.
- Organise family meetings to discuss care plans, progress, and any concerns or questions they may have.
Documentation and Reporting:
- Maintain accurate and up-to-date case notes, documentation, and reports in accordance with legal and regulatory requirements.
- Prepare comprehensive reports outlining clients' progress, recommendations for ongoing care, and any necessary changes to the care plan.
Qualifications:
- A degree in Social Work or a related field
- Proven experience working in brain injury rehabilitation units and care homes, preferably in a supervisory or leadership role.
- Strong knowledge of brain injury rehabilitation practices
- Excellent understanding of social care systems, community resources, and support services.
- Exceptional communication and interpersonal skills, with the ability to establish rapport and build trusting relationships with clients, families, and healthcare professionals.
- Strong organisational and time management skills, with the ability to prioritise and manage a caseload effectively.
- Self-motivated and able to work independently, demonstrating initiative and problem-solving abilities.
- Proficiency in maintaining accurate and detailed documentation.
