Keystone Case Management is recruiting for case managers in the London and South East.
We are clinically-led by a highly experienced team of registered healthcare professionals with an excellent reputation in the sector.
We work to improve the lives of people who have had catastrophic and life-changing events: adults and young people who have had a brain injury, amputation, poly trauma, complex fractures, birth injuries or spinal cord injury.

Established case managers welcome. Enhance your skills working with catastrophically injured clients; or in the supervision and development of newer case managers for which senior roles are available.
Purpose:
To work in partnership with clients and their families to support them in managing and improving their quality of life. This includes coordinating tailored rehabilitation, health and social care plans that allow individuals to lead positive and fulfilling lives. The support can be from acute injury, through rehabilitation and life-long.
Key responsibilities:
- Support, facilitate and advocate for the client and their family, in their own home community or other environment
- Provide holistic assessment of client needs, preparation of accurate, individualised reports which detail cost effective recommendations.
- Set goals with the client to progress their rehabilitation and life aspirations.
- Set up and manage packages for care.
- Research, arrange, coordinate, monitor and review healthcare, rehabilitation, educational and vocational services, both private and statutory, to ensure optimum client progress, outcomes and cost effectiveness.
- Conduct risk assessments and produce risk management plans.
- Effectively liaise with a wide range of health and social care and other services, including legal professionals, and work to a financial budget.
Qualifications and experience:
- Professional qualification in occupational therapy, social work, nursing, physiotherapy, psychology or speech and language therapy.
- Registered with HCPC, Social Work England, NMC or BPS.
- Minimum of 6 years' relevant post-graduate work experience.
- Community rehabilitation experience preferred.
- Knowledge of the medio-legal sector preferred.

Self-employed Case Manager opportunities are available for those with relevant case management experience or for Care Experts requiring a clinical role. Benefits will differ for those with self-employed status.
