AGENCY DESCRIPTION:
Agency on Aging Area 4 (AAA4) is a Joint Powers Authority (a Public Agency with non-profit status) formed between the counties of Nevada, Placer, Sacramento, Sierra, Sutter, Yolo, and Yuba to provide a system of supportive services for persons 60 years of age and over and people with disabilities. AAA4 funds and monitors a variety of programs for older adults in a seven-county planning and service area with funding received under the Older Americans Act and Aging and Disability Resource Connection (ADRC) state grants. AAA4 is one of 33 area agencies in California designated by the State to develop, coordinate, and fund programs designed to help older persons and people with disabilities maintain their health and independence.

PROGRAM DESCRIPTION:
The purpose of the Aging and Disability Resource Connection (ADRC) is to provide a single coordinated system for people seeking reliable information and access to Long-Term Services and Supports (LTSS). ADRCs function as a “No Wrong Door” single connection to enable people of all ages, incomes, and disabilities to access a wide array of LTSS options in the community. ADRCs in California are led by a core partnership between Independent Living Centers (ILC) and Area Agencies on Aging (AAA) and include a wide array of extended partner agencies. ADRCs provide 4 core services: Enhanced Information and Assistance, Short-term Crisis Intervention, Options Counseling, and Transition Support (hospital to home and nursing home to home).
POSITION SUMMARY:
Under the supervision of the Program Manager/Developer, the Case Manager/Navigator provides information and referral services, meets with consumers to complete service intakes, and provides options counseling and short-term service coordination using a person-centered approach. The Case Manager/Navigator also assists the Executive Director and Program Manager/Developer in coordinating with other ADRC partners to implement a “No Wrong Door” system of service delivery for people with disabilities and older adults. This position may require working at a satellite office and/or a partner agency.
EXAMPLES OF PRIMARY DUTIES/RESPONSIBILITIES:
Case Manager/Navigator I/II
- Assist individuals with immediate needs to navigate access to a wide range of public and private services and supports
- Conduct conversations and risk assessments to identify individual goals, strengths, values, preferences, and individual circumstances
- Assist with development of a person-centered plan to link the individual to appropriate resources
- Provide “warm handoffs” to streamline access to appropriate agencies, organizations, services, and programs
- Obtain, evaluate, and document comprehensive information related to community-based long-term care services
- Work collaboratively with public and private community partners to develop and sustain a “No Wrong Door” system for individuals to access LTSS
- Conduct outreach to enhance community awareness of LTSS options and the services of the ADRC
- Maintain accurate, timely, comprehensive, and confidential records
- Attend meetings including those of the advisory and steering committees
- Participate in periodic quality assurance and program evaluation efforts to sustain and advance program standards and effectiveness
- Assist with marketing, promoting, and conducting community outreach efforts to advance awareness of the program
Case Manager/Navigator II (Additional duties and responsibilities)
- Functions as the Leadworker over the Case Manager/Navigator team
- Coordinates the one-time and recurring training of the Case Manager/Navigators I
- Provides support and guidance to the Case Manager/Navigators I on an ongoing basis
- Maintain logs and other records needed to document and assess the various evidence-based programs conducted by/through the Agency
- Maintain and update protocols for the successful delivery of evidence-based, person-centered and transition programs
- Participate in creating such reports as are needed/required as part of the delivery of Agency programs
JOB CHARACTERISTICS:
The Case Manager/Navigator must equally be an active and patient listener; a creative and persistent problem-resolver; and an advocate for “No Wrong Door” approaches to LTSS. This position requires the skill to help the consumer to define his/her/their needs and to diplomatically gain his/her/their acceptance of the appropriate options available to best meet those needs. This position requires energetic and extensive exploration of community options and the ability to build effective relationships with community partners and various stakeholders. The Case Manager/Navigator II must find satisfaction in serving as a trainer and mentor to coworkers to assure their success in serving the consumers.
