Are you passionate about supporting our clients on their journey to recovery?
As the case coordinator your focus will be line managing A&R workers to carry out comprehensive assessments for those at imminent risk of rough sleeping and helping to reconnect them with supported networks in their home area or where they have a proven link. We’ll also rely on you to provide each client with personalised case management support and turn their assessment into a practical action plan that fully involves them.
In this role you will:
- Coordinate the client journey from rough sleeping to staying with us and most importantly finding them the right housing pathway in 28 days.
- You will manage the workload of up to 4 assessment and reconnection keyworkers.
- Liaise with outreach and other referrers within the westminster pathway to accept clients into the Harrow Road assessment Centre
- Attend move on related meetings held by the pathway commissioners
- Attend professionals meetings to advocate for clients in relation to their support needs and or move on
About you
We are looking for someone with:
- Experience of communicating, liaising and negotiating with internal and external to a high level.
- Experience of client case management and the resilience to achieve and meet agreed outcomes.
- The ability to manage complex behaviours and motivate staff and clients to achieve outcomes with a creative and person centred approach.
- The ability to coordinate a project or team in a homelessness service and provide reconnection advice.
- Excellent communication skills and the ability to work with others whilst managing your own time.
