Care Navigation is part of Lancashire County Council which is the fourth largest local authority in England covering a large geographic area and a wide range of communities. Our remit is to source Care in the home for the people of Lancashire.
If you are an enthusiastic and committed individual who is also caring and compassionate and you would genuinely like to contribute to helping those in need then the role of Care Navigation Officer could be for you. The ideal candidate will be highly motivated, organised and used to working with accuracy and attention to detail in a very fast paced environment. No care sourcing experience is necessary as on the job training will be provided.
We are looking to recruit several full-time permanent Care Navigation Officers. Successful applicants will be primarily working from home with occasional days in the Preston office, as required.
- Source care effectively for Lancashire residents using relevant IT systems and software
- Effectively manage all allocated caseloads showing commitment and enthusiasm for the role
- Maintain excellent communication skills with internal and external customers and colleagues using a variety of media.
- Follow processes and procedures adhering to strict timescales, escalating cases where necessary
- Maintain timely, accurate and up to date records and information for management and other professionals
- Work as part of a team to ensure service targets are met
- Work closely with colleagues, managers and other professionals to deliver an effective and high quality service
The service is open 364 days of the year (closed Christmas Day). The successful candidate will participate in a rota to cover working hours between 8am – 6pm Monday to Friday. Weekend and bank holiday working is between 9am – 5pm also on a rota basis.
