We are looking for an experienced Care Manager to join our team. The successful candidate will be responsible for providing comprehensive care management services to clients, including assessing needs, developing care plans, coordinating services, and monitoring outcomes. The Care Manager must be able to communicate effectively with clients and their families, as well as with other healthcare professionals. The ideal candidate will have experience in the healthcare field and a passion for helping others.
Responsibilities:
- Develop and implement individualised care plans for clients.
- Monitor client progress and adjust care plans as needed.
- Assess the needs of clients and their families.
- Provide emotional support to staff, clients and their families.
- Coordinate services with other health care providers.
- Maintain accurate records of client care plans and progress.
- Advocate for clients and their families in the health care system.
- Conduct audits.
- Carry out supervisions and appraisals.
- Regularly review policies and procedures.
- Ensure that our service is always following CQC requirements.
Job Types: Full-time, Permanent
