Job description
Certified Medical Assistant, Licensed Practical Nurse, Registered Nurse needed for Chronic Care Management with a Primary Care Practice. Responsibilities included: Care Management, contacting patients, making referrals, ordering patient services, communicating with physicians and other health service providers. Experience using EHR required. Experience using eClinicalworks. Must be comfortable making calls to patients. Ability to utilize technology required. Remote.
