Job description
We are recruiting for permanent CHAPs to work alongside our Registered Nurses, ensuring optimum levels of person centred care is delivered consistently to all residents. You will be supporting the Home Manager to deliver service levels within set budgets and providing continued support to other colleagues enabling them to contribute fully to the care team overall.
As a Care Home Assistant Practitioner you will be:
- Assisting the registered Nurse and consistently monitoring the standard of care being delivered to all residents
- Assisting the registered Nurse in ensuring that all residents receive quality medical and personal care
- Responsible for monitoring the wellbeing of each resident, being aware of any changes which impact on care and care plans
- Supporting the Home Manager in the implementation of all policies, procedures and business objectives along with preparing staff duty rota’s
- Identify training needs of the team and support with the implementation of the home’s training programme
- Conducting staff supervisions and appraisals ensuring they meeting external regulator guidelines
- Acting as a focal point for contact with residents, their relatives and visitors, as well as health and social care professionals

- Passionate about providing quality care in our homes
- Experienced in a relevant care role and able to evidence full competence
- Qualified in, or working towards, a level three health and social care qualification
- Committed to ongoing professional development
- Up to date with all personal essential learning
