The Role
This is an exciting opportunity to make a real difference to CYP in North Lanarkshire who are care experienced. The successful candidates will be part of the Specialist Children's Health Services Unit. This will support them to work collaboratively with colleagues and wider partners to make a real difference.
The main role the Care Experienced Young Persons Nurse - Throughcare and Aftercare is to provide caring and compassionate interventions that are underpinned by Scotland’s Promise to young people who are identified as needing additional support as they move towards adulthood. The foundations of the Promise will facilitate the role of these nurses to provide a scaffold of support to young people who are linked with North Lanarkshire’s Through and Aftercare Teams.
Care will be specifically focussed on health interventions that can maximise improvements in short and longer term outcomes for the young people identified. These interventions will be delivered as part of a whole system compassionate response to what the young person identifies that they need to grow and develop into healthy adulthood.
The post will support the foundations of the Promise and young people in receipt of Throughcare and Aftercare. A primary function of this role would be to provide the scaffolding of help and support to assist care leavers in achieving the best possible outcomes and transition successfully into adulthood. Care will be supported with view to improved assessment of health needs and review of any support pathways and promote self-efficacy. For the young people identified, aged up 25 years (until 26th Birthday). The post holder will work in close partnership with the Throughcare and Aftercare team and all identified partners and agencies.
The post holder will be responsible and accountable for maintaining all clinical, staff and information governance.

What we'll need you to bring
- Registered nurse at degree level or equivalent
- Post-registration qualification in public protection / child protection/Sexual Health/School Health or Health Visiting Qualifications & Knowledge

Educated to Masters level in a relevant nursing area, or
- Can demonstrate an equivalent level of knowledge gained through, for example, completion of NES modules on Assessment, Leadership, and Critical Thinking.
- Can provide advice using significant understanding and application of legislation relating to information sharing, data storage and public protection
- Knowledge of health and social care pathways related to safeguarding, early intervention and protection Knowledge of current legislation underpinning practice eg UNCRC, Getting it Right for Every Child and The promise

- Can demonstrate the following skills and attributes:
- Experience of caseload management and planning to meet deadlines
- Experience of using a strength based approach working in partnership with families
- Able to use a number of assessment tools used to decide referral pathways
- Management of risks and concerns using critical thinking to enable effective decision making
- Ability to provide professional advice within a multi-agency setting that puts children’s interests and safety first
- Can lead discussions and make decisions based on an assessment of a range of factors in complex situations
- Ability to develop and implement policies, procedures, service standards and improvements
- An ongoing commitment to continuing professional development
- The post holder should fulfil the NHS Scotland core values of care, compassion, dignity, respect, openness, honesty, responsibility and teamwork.

Please note this is a fixed term post for 2 years. Where a post only has temporary funding and an existing member of NHS Lanarkshire staff wishes to apply, this will be treated as a secondment. The employee MUST already have written agreement from their line manager to be released on a secondment before applying for the post.
Location and Working Pattern
This role will be based in SCHSU/School Nurse Team within Coathill Hospital.
The working pattern for this role Mon- Fri( 9-5). May need to work flexibly into evening to meet needs as and when
