The Forum Health Centre is looking for two Care Coordinators to join our friendly practice team.
The Forum Health Centre is located in Wyken, Coventry and is rated outstanding by the CQC. As a practice we take pride in providing healthcare services to our local population.
Our team of Care Coordinators are at the heart of the practice and are responsible for delivering high quality customer service to over 20,000 patients.
The hours for this post can be between 20 and 37.5 per week and usually Monday-Friday between 8am and 6:30pm.
Key responsibilities of the role include: booking and rescheduling patient appointments, answering calls to the practice, supporting patient queries, registering new patients, liaising with hospitals and other healthcare providers, supporting the clinicians with administration queries.
We are looking for two caring and conscientious individuals who demonstrate excellent communication and organisational skills. The right candidate must be able to work confidently in a busy environment as part of a team.
Previous customer service experience is required. Previous GP practice experience is desirable but not essential. Full Care Coordination training will be provided.

- GP Practice: 1 year (preferred)
- Customer Service: 1 year (required)
