We are looking for a motivated and driven individual to join our Home Care Service within the role of Care Coordinator. The role involves working within a team to schedule customer visits but also to support customers in their care provisions to include arranging GP visits and care planning.
Care Coordinator responsibilities include:
- Consulting with service user and family members to discuss their health problems.
- Educating service user about their condition, medication and giving them specific care and treatment instructions if required as this would be mainly done by the Care Manager.
- Developing a person centred care plan to address service user’s personal health care needs and updating care plans after reviews or hospital discharge if required.
- To attend emergency care calls in case the carers have emergency and can’t attend the care calls.
- To work with new recruits to monitor their competency level through coaching.
- To troubleshoot any care call related issues.
- To work and attend calls as per schedule.
- To liaise with various stakeholders (e.g. GP, Pharmacy, NHS Professionals) to coordinate service user receives effective, safe and quality care.
- To be on call as and when required.
- To report care related all queries to registered manager.
- To provide a detailed handover on daily basis to registered manager & admin coordinator.
- To provide supervision of all carers and facilitate all appraisals with registered manager.
- To ensure carers have reviewed adequate training and policy guidance.
- To work closely with admin team to ensure cares are fully compliant with HR related documents.
- To determine service user’s needs, developing an individualized plan of action that includes coordination from all parties involved in order to provide the best possible quality care.
To upload new service user’s details, their support plan & purchase order on the same day of new admission into the online portal and relevant systems.

Desirable Skills and Experience:
- At least one year's experience as the Care Coordinator of a domiciliary care agency.
- Previous experience as a Senior Care worker
