Biologics Home Care Nurse (Full Time)
Monday to Friday (no week end work)
Geographical areas : East Lancashire / Preston / Liverpool / Manchester

Role Summary
To deliver a Patient Support Programme for patients who have been initiated onto a biologics treatment.
The purpose of this role is to improve patient outcomes, supporting the initiation of a biologics treatment in a timely, safe manner and developing, deploying and coordinating the patient’s Care Plan in line with their needs
Key Aims and Responsibilities
- To work effectively and flexibly to deliver a Patient Support Programme to new and existing patients, and to coordinate and re-engage with patients who are ‘on hold’.
- To deliver the service to enrolled patients both face to face, through homecare visits, in clinics, and via telephone and virtual calls.
- Assist in the coordination of the prescription process. Working closely with Trust pharmacies to minimise delays in patients receiving treatment. Building relationships with the Pharmacists is key.
- To deliver, or administer or train patients on any diagnostic tests that may be required as the programme develops.
- Responsible for developing, agreeing and coordinating the patients individual Care Plan. To include the ongoing assessment, product and disease education, implementation and evaluation of the patient needs and capturing efficacy measures.
- Provide advice and education on treatment, plus disease management and lifestyle recommendations.
- Ensure that patients are treated with dignity, equality and respect and ensure that patients’ questions
- Record all patient data using the CRM system and ensure all administration is completed in a timely and accurate manner.
- To manage and implement initiation and re-initiation clinics for patients and give advice on which device will best fit the patient needs and lifestyle.
- Working professionally and effectively to build relationships with all stakeholders (clients, NHS consultants, Pharmacists, Nurses, other MDTs and internal staff).
Essential Requirements
- Current NMC Registration with specific experience in biologics administration (min Band 6).
- Biologics experience is essential. Experience of working in rheumatology, gastroenterology and/or dermatology preferred.
- Current U.K. full driving licence with no more than 6 points and an ability and willingness to travel to Trusts and patients’ homes within an acceptable geography.
- Excellent interpersonal skills and communication skills
- High levels of confidence, initiative & proactivity
Knowledge and Skills
- IT literate with experience of MS office specifically MS Teams, Excel, general experience of using computerised patient management systems.

Salary: £38,000.00-£40,000.00 per year
