Social prescribing enables health and care professionals to refer people to a range of local, non-clinical services to support their health and wellbeing. Transformation Partners in Health and Care have played a pivotal role in the development of policy, strategy, innovation, and operational delivery of Social Prescribing across London over the last 5 years. We support London's health and care systems to scale community centred models of care which focus on addressing the wider social determinants of health and target those experiencing the greatest health inequalities.

The post holder will take a lead on one or more workstreams and several projects within the programme, and be responsible for scoping, planning and delivery whilst engaging partners at every step of the way. The post holder will be expected to proactively manage and prioritise the work whilst spotting new opportunities for our programme to have the most impact on health inequalities. The post holder will develop and maintain relationships with key stakeholder in ICBs, VCSE, LA and ensure alignment between pan-London deliverables and local priorities.

We are looking for someone who can flexibly manage multiple projects at once and ensure they are impactful, whilst proactively supporting the broader strategic vision.

  • Lead and drive forward multiple projects within the programme ensuring they are planned, managed, and evaluated in a timely and effective way to ensure high quality of delivery across their portfolio of projects.
  • Responsible for proactively identifying and reporting on issues and risks and undertaking mitigating actions.
