An exciting opportunity has arisen to join Birmingham Community Healthcare Trust's, Virtual Ward services.
The purpose of the role is to work across community disciplines of nursing, occupational and physiotherapy, carrying out assigned tasks involving direct care as part of a Virtual Ward.
This role will involve working with patients as part of a multidisciplinary team to provide care to ensure the patient remains in their home.
The post holder will undertake implementation and evaluation of patient care needs for a defined group of patients under the delegated responsibility and indirect supervision of a registered professional.
The post holder will be expected to work shifts between the hours of 0800-2200 as part of a 7 day a week rota.

- The post holder will work alongside a registered practitioner providing support to ‘double up’ visits particularly for patients who have fallen and require two practitioners to assess and assist to a safe place.
- The post holder will provide support to the registered practitioners and undertake clinical interventions commensurate with the role such as phlebotomy, venepuncture, taking observations and completing an ECG in preparation for a registered practitioner to assessor review.
- Safeguard individuals from abuse by reporting any incidents involving potential or actual abuse immediately.
- Take responsibility for care and welfare of a defined caseload of patients within the service.
- Contributing to risk management by reporting all clinical and non-clinical incidents using the appropriate trust documentation.
- Complete an incident reporting tool (Datix) following any incidents.
- Carry out appropriate joint visits with other members of the multidisciplinary team.
- Implement care following a plan as agreed and delegated by a registered professional, and evaluate treatment, using appropriate programmes/ methods.
