The starting salary for this role is £26,569 per annum based on a 36-hour working week.
We are excited to be hiring two full-time permanent Social Care Assistants to join our fantastic teams, one at each of our two ASC Guildford locations:
- Guildford Locality team based at Guildford Borough Council, Millmead, Guildford, Surrey, GU2 4BB
- Royal Surrey County Hospital, Egerton Road, Guildford, Surrey, GU2 7XX

About the role
Royal Surrey County Hospital Team
No two days will be the same! You will work mainly based within Royal Surrey Hospital but also potentially between all of the hospitals. You will receive excellent support and supervision combined with comprehensive training/induction. You will work closely with the multi-disciplinary team to establish discharge plans and follow up on patients following discharge to reassess their ongoing needs, such as a variety of physical and cognitive impairments. You will also be involved in supporting safeguarding investigations and attending safeguarding meetings with the support of qualified practitioners. Social Care Assistants will have an experience of working with people with social care needs. You will need to have competent basic IT skills and be able and willing to learn to use a variety of software systems.
Guildford Locality Team
In this role you will have frequent direct contact with the public on the phone as well as visiting them in their own home or care setting. Your role will be to assist qualified practitioners with a variety of tasks, including assessing people’s needs and carrying out regular reviews.
You will frequently be the first point of contact between the public and the service, gathering initial information and identifying the need for further assessment, as well as signposting people to other services.
You will work in partnership with individuals, their carers, professional staff in other agencies, service providers and Support Brokers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control. Social Care Assistants will hold a caseload of individuals and carers who will generally have a support plan in place that requires routine monitoring.

- Your IT skills and ability and willingness to learn to use a variety of software systems.
- Your ability to communicate in a clear, fluent, concise and jargon-free way and in a courteous, calm and professional manner.
You will be working in a team of motivated, dedicated and friendly colleagues and have an added bonus of working closely alongside a wider multi-disciplinary team in a thriving hospital environment.
The work involves travelling within South-West Surrey and also to other parts of the county. We therefore ask that you are a car driver and have access to a car.
