Job description

This role is to provide assurance that safe and effective services are being provided for people with a learning disability and/or autism. It will involve participating in the assessment, planning and implementation of pro-active care and case management of people from across Lancashire and South Cumbria population with a Learning Disability and or Autism. You will be the main point of contact for health and social care colleagues as well as providers, service users, family and carers in relation to the appropriateness of placements and facilitating people to move through the system.
