Are you looking for a new challenge where you can make a difference in the lives of vulnerable young people aged 16-25 who are at risk of or are not in education, employment or training? Young people who are at risk of homelessness, substance misuse and have complex needs. We have an exciting opportunity for a Nurse, Occupational Therapist or Social Worker to join Surrey & Borders NHS Foundation Trust (SABP) in the newly transformed Reaching Out service as part of the Assertive Engagement Pathway. The Reaching Out service focuses on engaging and supporting the most isolated and vulnerable children and young people through a multi-agency network. We are looking for a clinician who is creative, highly motivated, shows initiative and passionate and committed for ensuring high quality services for people with mental health needs. There is 1 part-time post covering the West of Surrey with SABP bases across the county. To manage a caseload of young people referred to the service, working collaboratively with other services both internal and external within a borough/locality to provide consultations and mental health assessments to assist young people overcome the obstacles they face and to achieve their potential. To build a trusting relationship to develop a deeper understanding of the needs of young people and be able to signpost or refer to more appropriate services. This role will involve outreach into the community to meet young people in their own or choice of environment. To work in partnership with young people, families/ carers and other services regarding decision making and planning to help them to make informed decisions, to enable them to clarify and express their needs and to co-produce solutions. We also provide social care services for people with a learning disability in Croydon and ASD and ADHD assessment services in Hampshire.
